In-Home Physician Per Diem - Zanesville
About The Position
Signify Health, part of CVS Health, is seeking a per diem physician to provide In-Home Health Evaluations in members’ homes and help bridge gaps in care. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and a clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s current treatment regimen. The role may also include virtual visits and locally or regionally required travel within licensed states. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The position is part-time and per diem, with an anticipated 12 hours per week. Compensation is an hourly base rate plus possible incentives. The role includes malpractice coverage for Signify Health-related activities, supplies, and other perks.
Requirements
- Active, unrestricted physician license(s) in the coverage area(s).
- Ability to comfortably and safely perform duties in varied home environments, including homes with pets, secondhand smoke, or other potential airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
- Perform additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
- Conduct virtual visits as needed.
- Travel locally and within licensed states as needed.
- Build trusted relationships with members and help connect them with appropriate care.
- Fulfill the requirements of the specified clinician role.
