About The Position

Signify Health, part of CVS Health, is seeking a per diem physician to provide in-home health evaluations for health plan members. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a person’s current treatment regimen. The role may also involve virtual visits and providing diagnostic and preventive services. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. Local and in-state travel may be required.

This is a part-time, per diem position with approximately 12 anticipated weekly hours. Compensation is an hourly base rate with potential additional incentives. The stated pay range is $105.00–$105.00 per hour. Per diem employees qualify for malpractice insurance coverage during Signify Health-related activities, supplies, and other perks. This part-time position may be eligible for benefits and programs including dental, vision, wellness resources, employee discounts, and certain voluntary benefits, depending on hours worked and eligibility.

Requirements

• Active, unrestricted physician license(s) in the coverage area(s).
• Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations for health plan members, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by the member’s health plan, such as diabetic eye exams or spirometry.
• Use an iPad and the clinical workflow for evaluations.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
• Travel locally and within licensed states as needed.
• Build trusted relationships with members and help connect them with appropriate care.
