About The Position

Signify Health, part of CVS Health, is seeking a per diem physician to provide in-home health evaluations for health plan members. The role focuses on building trusted relationships, helping people address gaps in care, and delivering flexible, comprehensive, and personalized health evaluations in members’ homes. Visits typically last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a member’s existing treatment regimen.

The position involves local travel and travel within licensed states as needed, as well as virtual visits when required. The primary workplace is members’ private residences, where conditions vary and may include household pets, secondhand smoke, or other airborne allergens or irritants. Physicians provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screenings.

This is a part-time, per diem position with anticipated weekly hours of 12. Compensation is an hourly base rate of $105, with additional incentives potentially available for completed visits and other activities. Per diem employees qualify for malpractice insurance coverage during Signify Health-related activities, supplies, and other perks. Depending on hours worked and eligibility, benefits may include dental and vision coverage, wellness resources, employee discounts, access to certain voluntary benefits, and other programs.

Requirements

• Hold an active, unrestricted medical license in the coverage area(s).
• Be able to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.
