About The Position

Signify Health, part of CVS Health, is seeking a per diem physician to provide in-home health evaluations to health plan members. Physicians meet with members in their homes for visits that last about 45 minutes, building trusted relationships and helping connect people to care. Evaluations include reviewing medications and medical history and conducting a physical evaluation. Additional diagnostic tests, such as diabetic eye exams or spirometry, may be included if ordered by the member’s health plan. Physicians use an iPad and a clinical workflow. They are not required to prescribe medication, order laboratory tests, or change a member’s current treatment regimen. The role may also involve virtual visits and requires local travel and travel within licensed states as needed.

The primary workplace is members’ private residences. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants; physicians must be able to perform duties comfortably and safely in these conditions. Per diem employees receive an hourly base rate plus potential incentives, malpractice insurance for Signify Health-related activities, supplies, and other perks. The part-time position is eligible for certain benefits and programs depending on hours worked and eligibility.

Requirements

• Active, unrestricted physician license(s) in the coverage area(s).
• Ability to safely and comfortably perform duties in members’ homes, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic and preventive services as indicated, such as spirometry, diabetic eye exams, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide Signify Health services in accordance with the requirements of the role.
• Help connect members with appropriate care and support improved health outcomes.
