About The Position

Signify Health, part of CVS Health, is seeking a full-time physician to provide in-home health evaluations for health plan members. Physicians conduct approximately 45-minute visits in members’ homes, building trusted relationships and helping connect people with appropriate care. Visits include reviewing medications and medical history, conducting a physical evaluation, and performing additional diagnostic tests when ordered by the person’s health plan. Physicians use an iPad and a clinical workflow and are not required to prescribe medication, order laboratory tests, or change a person’s current treatment regimen. Virtual visits may also be conducted as needed. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, and other airborne allergens or irritants. Local and in-state travel may be required.

Requirements

Active, unrestricted medical license(s) in the coverage area(s).
Ability to safely and comfortably perform duties in members’ homes, which may contain pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

Conduct in-home health evaluations and engage with members one-on-one about their health questions and needs.
Review medications and medical history and perform a physical evaluation during visits.
Provide additional diagnostic and preventive services as indicated, such as spirometry, diabetic eye exams, and bone density screening.
Conduct virtual visits as needed.
Travel locally and within licensed states as needed.
Fulfill the requirements of the specified clinician role and contribute to efforts to improve health outcomes and connect members to appropriate care.
