About The Position

Signify Health, part of CVS Health, is seeking a full-time physician to provide in-home health evaluations to health plan members in Indianapolis. Physicians conduct approximately 45-minute visits in members’ homes, connecting one-on-one, answering health questions, reviewing medication and medical history, and performing physical evaluations. Additional diagnostic tests may be included when ordered by the member’s health plan. Physicians use an iPad and clinical workflow and are not required to prescribe medication, order lab tests, or change current treatment regimens.

The role involves delivering Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screenings. Physicians may conduct virtual visits and must be flexible to travel locally and within licensed states as needed. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants; clinicians must be able to perform their duties comfortably and safely in these environments.

Requirements

• Hold an active, unrestricted medical license in the coverage area(s).
• Be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Answer members’ health questions and build trusted relationships during one-on-one visits.
• Provide additional diagnostic tests when ordered by the member’s health plan.
• Use an iPad and the clinical workflow for each evaluation.
• Provide Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screenings.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Fulfill the requirements of the specified clinician role and contribute to efforts to connect people with appropriate care.
