In-Home Physician Full Time - Davenport
About The Position
Signify Health, part of CVS Health, is seeking a full-time Physician to provide in-home health evaluations and help bridge gaps in care. Physicians conduct visits of about 45 minutes in the homes of health plan members, using the time to connect one-on-one and answer health questions. Evaluations include a medication and medical history review and a physical evaluation, along with additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medicine, order lab tests, or alter a person’s current treatment regimen. The role is part of a national network of clinicians working to improve health outcomes and connect people with care suited to their needs. The physician may travel locally and within licensed states, conduct virtual visits as needed, and provide Signify Health services, including diagnostic and preventive services. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to perform their duties comfortably and safely in these settings.
Requirements
- Active, unrestricted physician license(s) in the coverage area(s).
- Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities
- Conduct in-home health evaluations lasting approximately 45 minutes.
- Connect one-on-one with health plan members and answer their health questions.
- Review members’ medications and medical histories and conduct physical evaluations.
- Perform additional diagnostic tests when ordered by the member’s health plan.
- Use an iPad and the clinical workflow for each evaluation.
- Travel locally and within licensed states as needed.
- Conduct virtual visits as needed.
- Provide Signify Health services as indicated, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
- Help connect people to care suited to their needs and contribute to improved health outcomes.
