About The Position

Signify Health, part of CVS Health, is seeking a part-time physician to provide in-home health evaluations for health plan members. Visits last about 45 minutes and include a one-on-one conversation, medication and medical history review, and physical evaluation. Additional diagnostic tests, such as diabetic eye exams or spirometry, may be provided when ordered by the member’s health plan. Physicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change a person’s current treatment regimen. The role may also include virtual visits and local travel within licensed states. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

The position is part of a national network of clinicians focused on improving health outcomes and connecting people with appropriate care. Physicians may make referrals to a member’s health plan, encourage loved ones’ participation, and ensure primary care providers are informed about members’ health and quality of life.

Requirements

• Active, unrestricted physician license(s) in the coverage area(s).
• Ability to safely and comfortably perform duties in private homes that may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations for health plan members.
• Review members’ medications and medical histories and perform physical evaluations.
• Provide diagnostic and preventive services as indicated, including services such as spirometry, diabetic eye exams, and bone density screening.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Help connect members with appropriate care and communicate relevant health information to primary care providers.
