About The Position

Signify Health, part of CVS Health, is seeking a part-time physician to provide in-home health evaluations and help bridge gaps in care. Physicians visit people in their homes for approximately 45 minutes, allowing time for a one-on-one conversation and questions. Evaluations include reviewing medications and medical history, conducting a physical evaluation, and performing additional diagnostic tests when ordered by the person’s health plan, such as diabetic eye exams or spirometry. Physicians use an iPad and an intuitive clinical workflow. The role does not require prescribing medicine, ordering lab tests, or changing a person’s current treatment regimen.

The position involves local travel and travel within licensed states as needed, as well as virtual visits when needed. Physicians provide Signify Health services as indicated, including diagnostic and preventive services. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. Physicians must be able to perform duties comfortably and safely in these environments.

The position is part time, with 29.5 anticipated weekly hours. The stated pay range is $105.00 per hour. Requirements

• Hold an active, unrestricted physician license in the coverage area or areas.
• Be able to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
• Perform additional diagnostic tests when ordered by a person’s health plan, including services such as diabetic eye exams and spirometry.
• Use an iPad and clinical workflow for each evaluation.
• Engage with people one-on-one, answer health questions, and help connect them with care suited to their needs.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Signify Health services as indicated, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
• Fulfill the requirements of the specified clinician role.
