In-Home Physician Full Time - Zanesville

About The Position

Signify Health, part of CVS Health, is seeking a full-time Physician to provide In-Home Health Evaluations and help bridge gaps in care. Physicians visit people in their homes, building trusted relationships and providing flexible, comprehensive, and personalized health evaluations. Each visit lasts about 45 minutes and includes time for a one-on-one conversation, answering health questions, reviewing medication and medical history, and conducting a physical evaluation. Additional diagnostic tests may be provided when ordered by the person’s health plan. Physicians use an iPad and an intuitive clinical workflow. The role does not require prescribing medicine, ordering lab tests, or changing a person’s current treatment regimen. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. The role also includes local travel and travel within licensed states as needed, virtual visits as needed, and provision of Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.

Requirements

Active, unrestricted physician license(s) in the coverage area(s).
Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

Conduct approximately 45-minute In-Home Health Evaluations, including medication and medical history reviews and physical evaluations.
Engage with people one-on-one, answer health questions, and build trusted relationships to help improve health outcomes.
Help connect people with appropriate care for their individual needs and help bridge gaps in care.
Use an iPad and the clinical workflow for each evaluation.
Travel locally and within licensed states as needed.
Conduct virtual visits as needed.
Provide Signify Health services as indicated, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
Work as a clinician colleague and fulfill the requirements of the specified role.
