About The Position

Signify Health, part of CVS Health, is seeking a full-time physician to provide in-home health evaluations and help bridge gaps in care. Physicians visit health plan members in their homes for approximately 45 minutes, allowing time for one-on-one conversation and questions. Evaluations include a medication and medical history review and a physical evaluation, plus additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and an intuitive clinical workflow. The role does not require prescribing medicine, ordering lab tests, or changing a member’s current treatment regimen. The physician joins a national network of clinicians working to improve health outcomes and connect people with care suited to their needs. The role may involve local travel and travel within licensed states, as well as virtual visits as needed. Physicians provide Signify Health services as indicated, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening. The primary workplace is the private residence of health plan members. Home environments vary and may include household pets, secondhand smoke, and other airborne allergens or irritants. Candidates must be able to comfortably and safely perform their duties in these environments.

Requirements

Active, unrestricted medical license(s) in the coverage area(s).
Ability to comfortably and safely perform duties in home environments that may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities

Conduct in-home health evaluations, including medication and medical history reviews and physical evaluations.
Answer members’ health questions and build trusted relationships through one-on-one visits.
Perform additional diagnostic tests when ordered by the person’s health plan.
Use an iPad and the clinical workflow for each evaluation.
Travel locally and within licensed states as needed.
Conduct virtual visits as needed.
Provide indicated Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening.
Help connect people to care appropriate to their needs and support efforts to improve health outcomes.
