About The Position

Signify Health, part of CVS Health, is seeking a full-time Physician to provide in-home health evaluations to health plan members. The role focuses on building trusted relationships, helping people address gaps in care, and providing flexible, comprehensive, personalized evaluations in members’ homes. Visits last approximately 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the person’s health plan. Physicians use an iPad and clinical workflow and are not required to prescribe medication, order laboratory tests, or change a person’s current treatment regimen. Virtual visits may also be conducted as needed. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, and other airborne allergens or irritants. The position requires local travel and travel within licensed states as needed. Full-time employees are eligible for a comprehensive benefits package.

Requirements

Active, unrestricted medical license(s) in the coverage area(s).
Ability to comfortably and safely perform duties in members’ homes, which may contain household pets, secondhand smoke, and other airborne allergens or irritants.

Responsibilities

Conduct in-home health evaluations and connect with members one-on-one to answer health questions.
Review members’ medication and medical history and perform physical evaluations.
Provide additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
Use an iPad and the clinical workflow for each evaluation.
Travel locally and within licensed states as needed.
Conduct virtual visits as needed.
Provide Signify Health services as indicated.
Help connect people with appropriate care and support improved health outcomes.
