About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide in-home health evaluations and help bridge gaps in care. Clinicians visit health plan members in their homes, build trusted relationships, and provide flexible, comprehensive, and personalized evaluations. Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and clinical workflow and are not required to prescribe medicine, order lab tests, or change a person’s current treatment regimen. The role may also include virtual visits and local travel, including within licensed states as needed.

The primary workplace is members’ private residences. Home environments vary and may contain household pets, secondhand smoke, or other airborne allergens or irritants; clinicians must be able to perform their duties comfortably and safely in these conditions. The position is full time, with 40 anticipated weekly hours. The stated pay range is $87,035 to $187,460; actual compensation depends on factors such as experience, education, and geography.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.

Responsibilities

• Provide in-home health evaluations to health plan members.
• Review members’ medications and medical histories and conduct physical evaluations.
• Perform additional diagnostic tests when ordered by a member’s health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide Signify Health services, including Diagnostic & Preventive Services products as indicated.
• Help connect members to appropriate care for their individual needs and answer their health questions.
