About The Position

Signify Health, part of CVS Health, is seeking a full-time Nurse Practitioner or Physician Assistant to provide In-Home Health Evaluations in Peoria. Clinicians meet with health plan members in their homes to build trusted relationships, help bridge gaps in care, and support better health outcomes. Visits typically last about 45 minutes and include a medication and medical history review, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Clinicians use an iPad and an intuitive clinical workflow. They are not required to prescribe medication, order lab tests, or change members’ current treatment regimens.

The role may also include virtual visits and locally or in-state travel as needed. The primary workplace is members’ private residences, which may have household pets, secondhand smoke, or other airborne allergens or irritants. Candidates must be able to perform their duties comfortably and safely in these environments.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification.

Responsibilities

• Conduct in-home health evaluations, typically lasting about 45 minutes.
• Review members’ medication and medical histories and perform physical evaluations.
• Perform additional diagnostic tests when ordered by the member’s health plan, including services such as diabetic eye exams, spirometry, or bone density screening.
• Build trusted relationships with members and help connect them to care suited to their needs.
• Conduct virtual visits as needed.
• Travel locally and within licensed states as needed.
• Provide Signify Health services, including Diagnostic & Preventive Services products.
