About The Position

Signify Health, part of CVS Health, is seeking a part-time physician to provide in-home health evaluations and help bridge gaps in care. Physicians conduct visits lasting about 45 minutes, using the time to connect one-on-one with members and answer health questions. Evaluations include medication and medical history reviews, a physical evaluation, and additional diagnostic tests when ordered by the member’s health plan. Physicians use an iPad and an intuitive clinical workflow. The role does not require prescribing medication, ordering lab tests, or changing a person’s current treatment regimen.

The physician joins a national network of clinicians working to improve health outcomes and connect people with care suited to their needs. The role may require local travel and travel within licensed states, as well as virtual visits. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. Physicians must be able to perform their duties comfortably and safely in these varied home environments.

Requirements

• Active, unrestricted physician license(s) in the coverage area(s).
• Ability to comfortably and safely perform duties in members’ homes, which may contain pets, secondhand smoke, and other potential airborne allergens or irritants.

Responsibilities

• Conduct one-on-one in-home health evaluations, typically lasting about 45 minutes.
• Review members’ medication and medical history and perform a physical evaluation.
• Provide additional diagnostic and preventive services when indicated, such as spirometry, diabetic eye exams, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Help connect members with appropriate care and support improved health outcomes.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services indicated for the role.
