About The Position

Signify Health, part of CVS Health, is seeking a part-time physician to provide in-home health evaluations and help bridge gaps in care. Physicians conduct approximately 45-minute visits in health plan members’ homes, building trusted relationships and offering time for one-on-one conversations and health questions. Evaluations include a medication and medical history review and a physical evaluation; additional diagnostic tests may be performed when ordered by the person’s health plan. Physicians use an iPad and a clinical workflow. They are not required to prescribe medication, order lab tests, or change a person’s current treatment regimen.

The role involves local and in-state travel as needed, virtual visits as needed, and delivery of Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, and bone density screening. The primary workplace is members’ private residences, which may include household pets, secondhand smoke, and other airborne allergens or irritants. Physicians should be able to perform their duties comfortably and safely in these environments.

Requirements

• Active, unrestricted physician license(s) in the coverage area(s).
• Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations, typically lasting about 45 minutes.
• Review members’ medication and medical history and perform a physical evaluation.
• Perform additional diagnostic tests when ordered by the member’s health plan.
