About The Position

Signify Health, part of CVS Health, is seeking a per diem physician to provide in-home health evaluations and help bridge gaps in care. Physicians conduct visits of approximately 45 minutes in members’ homes, allowing time for one-on-one conversations and health questions. Evaluations include medication and medical history review and a physical evaluation, with additional diagnostic tests such as diabetic eye exams or spirometry when ordered by the person’s health plan. Physicians use an iPad and a clinical workflow. They are not required to prescribe medication, order laboratory tests, or alter a person’s current treatment regimen. The position may also include virtual visits and delivery of indicated diagnostic and preventive services. The primary workplace is members’ private residences, which may contain household pets, secondhand smoke, or other airborne allergens or irritants. Local and in-state travel may be required.

Requirements

• Active, unrestricted physician license(s) in the coverage area(s).
• Ability to safely and comfortably perform duties in private homes that may contain pets, secondhand smoke, or other airborne allergens or irritants.

Responsibilities

• Conduct in-home health evaluations and build trusted relationships with health plan members.
• Review members’ medications and medical histories and perform physical evaluations.
• Perform additional diagnostic tests when ordered by a member’s health plan, including services such as diabetic eye exams, spirometry, and bone density screening.
• Use an iPad and the clinical workflow for each evaluation.
• Conduct virtual visits as needed.
• Provide Signify Health services, including indicated diagnostic and preventive services.
• Travel locally and within licensed states as needed.
• Work as a clinician colleague in accordance with the requirements of the specified role.
