About The Position

The Nurse Practitioner provides on-site care at subacute rehabilitation facilities in Far Rockaway, Queens. The role focuses on helping patients improve mobility, manage pain, and regain independence while working with nursing and rehabilitation teams. Additional part-time opportunities are available in Nassau and Suffolk Counties on Long Island; locations may be combined to create a full-time role.

Requirements

Current Nurse Practitioner certification.
Active New York RN license and Nurse Practitioner certification.
Active DEA registration for prescribing controlled substances.
Experience in subacute rehabilitation, geriatrics, orthopedics, internal medicine, pain management, or a related setting.
Strong clinical assessment, documentation, and communication skills.
Ability to manage a patient panel and collaborate effectively with interdisciplinary teams.

Responsibilities

Perform comprehensive patient assessments and follow-up visits in subacute rehabilitation settings.
Manage rehabilitation-related medical needs, pain, and conditions affecting mobility and function.
Develop individualized treatment plans in collaboration with facility clinical teams and physicians, as applicable.
Prescribe and manage medications within the scope of practice.
Perform joint and trigger-point injections when appropriately trained and authorized.
Coordinate with physical, occupational, and speech therapy teams to support rehabilitation goals.
Complete accurate, timely documentation of treatment plans and patient progress.
Educate patients and families and support care transitions.
