About The Position

Requirements

• Graduate of an accredited school of nursing.
• Current LPN license in the community’s state.
• Minimum of three years’ experience in a Nurse Supervisor role.
• Two years’ experience in geriatric nursing and/or a Bachelor of Science degree in nursing.
• Minimum of two years’ nursing administration experience, preferably in a long-term care setting.
• Basic computer proficiency.
• Strong interpersonal and communication skills.
• Ability to communicate effectively with residents, families, staff, and visitors.
• Ability to maintain the highest level of confidentiality.

Nice To Haves

• Participation in professional organizations dealing primarily in geriatrics.
• Memory Care experience.

Responsibilities

• Directly supervise community-based licensed nursing staff (LPNs and RNs), Med Techs/CNAs where required by state regulation, lead caregiving employees, and potentially Resident Care staff.
• Assign and direct subordinate employees’ work; appraise performance; reward and discipline employees; and address complaints and resolve problems.
• Train, supervise, and monitor employees administering medications as allowed by the Nurse Practice Act, including auditing medication administration records.
• Provide ongoing training and education to resident-care employees through classroom in-services and situation-specific instruction.
• Supervise the maintenance of resident charts and review documentation completed by caregiving staff.
• Assess residents’ health, functional, and psychosocial status as allowed by the Nurse Practice Act; initiate individualized service plans; proactively manage care; evaluate effectiveness; and support residents’ ability to remain in their environment.
• Manage residents’ healthcare and disseminate relevant information to families and employees.
• Ensure family members are aware of residents’ scheduled appointments.
• Participate in pre-admission screening of prospective residents.
• Ensure required documentation is completed before or upon admission, including nursing assessments, service plans, and other assigned forms.
• Update assessments as required by policy and as allowed by the Nurse Practice Act.
• Conduct on-site evaluations of residents admitted to alternate care environments, maintain contact with their families, and support their return to the community.
• Continuously assess and observe residents’ physical and psychosocial needs and coordinate with other departments to ensure proactive, quality care.
• Evaluate residents, document changes in condition, and notify the executive director, physician, and legally responsible party or family of residents’ conditions and reactions.
• Communicate with physicians about residents’ status and changes in condition.
• Prepare residents for physician appointments, prepare Physician Visit Forms, and review and update resident charts.
• Coordinate continuity of care for residents receiving home health care, hospice, and other third-party healthcare services.
