About The Position

Requirements

• 1–2 years of customer service support experience in the healthcare industry.
• Effective verbal and written communication skills.
• Ability to use computer programs and applications, including EMR/EHR systems.
• Ability to self-motivate and work independently.
• Customer focus and adaptability to different personality types.
• Ability to multitask, set priorities, and manage time effectively.
• Familiarity with patient referrals.

Nice To Haves

• Home Health Services experience is preferred.

Responsibilities

• Process patient referrals and maintain client information in the database.
• Provide clerical and administrative support to DME Customer Service in daily care management activities.
• Support incoming and outgoing telephone calls involving patients, providers, Transition Coaches, and other team members about the Transition Care program.
• Expand, update, and maintain the community resource database.
