About The Position

St. Matthews Pharmacy is seeking a talented Specialty Pharmacist to join their team in Louisville, KY. This role is part of an innovative pharmacy program focused on removing barriers to treatment and delivering life-saving services and medications to patients in a stigma-free environment, particularly those affected by addiction. The high-touch specialty pharmacy program supports patients with complex medical needs, including behavioral health, mental health, and addiction treatment. The pharmacist will collaborate closely with prescribers, care teams, and patients to ensure safe, effective medication therapy and improved patient outcomes. This position offers the opportunity to practice at the top of your license through direct patient counseling, care plan management, and collaboration with providers. Requirements

  • Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy
  • Active, unrestricted pharmacist license (U.S.) with the Kentucky Board of Pharmacy
  • Strong clinical judgment and patient communication skills

Responsibilities

  • Provide pharmacist oversight for specialty medications, including biologics and limited-distribution drugs, ensuring safe, effective, and compliant use.
  • Conduct comprehensive clinical assessments and oversee patient onboarding for specialty therapy initiation.
  • Monitor patient therapy adherence, clinical response, side effects, and treatment outcomes; intervene as appropriate.
  • Identify, document, and report adverse drug events in a timely and compliant manner.
  • Maintain accurate and comprehensive prescription and patient records; develop, evaluate, and update patient care plans; and document clinical and non-clinical patient interventions across designated systems and platforms.
  • Coordinate and support the resolution of prior authorizations, Risk Evaluation and Mitigation Strategy (REMS) requirements, and third-party insurance approvals.
  • Educate patients and caregivers on proper medication administration techniques, storage requirements, and clinical safety considerations.
  • Support specialty pharmacy quality measures, adherence initiatives, and clinical program objectives.
  • Maintain accountability for the general supervision of pharmacy staff, ensuring appropriate oversight, training support, and adherence to established workflows.
  • Provide direct supervision of medication dispensing and preparation processes to ensure safety, accuracy, and regulatory compliance.
  • Ensure adherence to FDA mandates and manufacturer-imposed restrictions, including limited distribution and REMS program requirements, as applicable.
  • Administer medications via gluteal, intramuscular (IM), and subcutaneous injection routes, as permitted by scope of practice and regulation.
  • Ensure full compliance with all applicable regulatory, accreditation, and organizational requirements, including: Specialty pharmacy accreditation standards (e.g., URAC, ACHC), with responsibility for ongoing accreditation readiness, participation in audits and surveys, and support of continuous quality improvement initiatives; State Board of Pharmacy laws and regulations; Company policies, procedures, and operational standards.
  • Participate in a rotating on-call schedule to support pharmacy operations and patient care needs.
  • Perform additional duties as assigned in support of pharmacy operations, clinical services, and organizational objectives.
