Inpatient Pharmacist – Contract - IPDS

About The Position

This is an 8-week contract position for an Inpatient Pharmacist in Hood River, Oregon. The role involves day and night rotation shifts, working 40 hours per week. The pharmacist will be responsible for verifying and processing inpatient medication orders using EPIC, providing clinical review of medication therapy, ensuring safe and accurate dispensing, and participating in medication management and patient care activities. The position also requires responding to overnight clinical pharmacy needs and on-call requests, collaborating with healthcare teams, monitoring drug therapies, providing drug information, ensuring compliance with regulations, supporting medication reconciliation and discharge planning, and participating in weekend coverage. The role also involves maintaining accurate documentation and contributing to medication safety initiatives.

Requirements

  • Doctor of Pharmacy (PharmD) required.
  • Active Oregon Pharmacist License required.
  • Inpatient hospital pharmacy experience preferred.
  • Strong understanding of acute care pharmacotherapy.
  • Experience working in fast-paced hospital environments.
  • Ability to work both day and night shifts on a rotating schedule.
  • Strong clinical judgment and attention to detail.
  • Excellent communication and teamwork skills.

Nice To Haves

  • Prior experience in a Critical Access or Teaching Hospital.
  • Experience with EPIC electronic health record system.
  • Strong background in inpatient clinical pharmacy services.
  • Ability to manage high-acuity patient populations.

Responsibilities

  • Verify and process inpatient medication orders using EPIC.
  • Provide clinical review of medication therapy for hospitalized patients.
  • Ensure safe, accurate, and timely dispensing of medications.
  • Participate in medication management and patient care activities.
  • Respond to overnight clinical pharmacy needs and on-call requests.
  • Collaborate with physicians, nurses, and interdisciplinary care teams.
  • Monitor drug therapies for safety, effectiveness, and appropriateness.
  • Provide drug information and clinical consultation to healthcare staff.
  • Ensure compliance with hospital policies, state regulations, and pharmacy standards.
  • Support medication reconciliation and discharge planning processes.
  • Participate in weekend pharmacy coverage as required.
  • Maintain accurate documentation in pharmacy systems.
  • Contribute to medication safety and quality improvement initiatives.
