About The Position

Requirements

• Master of Social Work (MSW).
• Licensed Master Social Worker (LMSW) required.
• Three to five years of experience in community social work or hospital social work, with a good appreciation for community resources preferred.
• Comfort with frequent use of computer systems and strong knowledge of Microsoft Excel, Word, PowerPoint, and Outlook.
• Strong interpersonal and organizational skills and excellent written communication skills.
• Ability to communicate effectively with people from diverse backgrounds, listen to patient information, and communicate it to the team.
• Ability to collaborate effectively in a team setting and maintain professional relationships with patients and other care team members.
• Willingness to learn and carry out structured clinical protocols with patients in person or by telephone.
• Appreciation for patient confidentiality, accurate data collection, and professional communication across health care sectors.

Responsibilities

• Conduct intake and clinical assessments, including psychosocial assessments.
• Refer patients to community resources and conduct outreach.
• Coordinate care and provide care management services to patients and families in an outpatient setting.
• Provide brief behavioral health interventions as needed and psycho-educational support.
• Collaborate with practice teams to assist patients with identified social work needs.
• Consult with health care providers regarding patient care needs and community resources.
• Oversee the team of outpatient Social Workers under Highland Hospital.
