About The Position

Evergreen At Home is seeking a Registered Nurse to provide skilled home health nursing services to older adults in their homes throughout the community. The nurse will work with physicians, therapists, patients, and families to coordinate care, manage chronic conditions, and support patients’ quality of life and independence. The role offers hourly compensation of $37.50–$42.50, based on experience and number of shifts, and includes drive time and documentation time rather than per-visit pay. The schedule is 30–40 hours per week during the daytime, with flexibility to create a daily schedule. The driving radius is limited to no farther than 45 minutes from Oshkosh. The nurse participates in weekly team meetings to discuss caseload. Evergreen At Home is described as a nonprofit that reinvests in services, programming, equipment, and employees. Requirements

  • Active Wisconsin Registered Nurse (RN) license.
  • Current CPR certification.
  • Valid Wisconsin driver's license.
  • Reliable transportation, appropriate auto insurance, and a good driving record.
  • Strong organizational and time-management skills.
  • Passion for serving older adults and supporting them in maintaining independence.

Nice To Haves

  • Previous home health experience.
  • Experience managing patients with chronic medical conditions.
  • Familiarity with OASIS documentation.

Responsibilities

  • Provide skilled nursing care to older adult patients in their homes.
  • Coordinate care plans with physicians, therapists, and other healthcare providers.
  • Monitor patient conditions and promptly communicate changes.
  • Educate patients and family members about disease management, medications, and care plans.
  • Complete accurate OASIS documentation.
  • Assist with discharge planning and patient transitions of care.
  • Connect patients and families with community resources and support services as needed.
