About The Position

Vecinos is a non-profit, 501(c)3 Free Clinic that provides bilingual integrated primary and mental health care, mobile clinic services, health education, and case management to uninsured, low-income adults in the far-western NC counties. We offer a positive, supportive, and team-oriented atmosphere. We work in a non-traditional environment with limited resources. Schedules and hours vary throughout the year. The Registered Nurse helps chronic disease patients — with special focus on mobile clinic patients — understand, access, and stay on their prescribed medications. Working within the Chronic Disease Management (Care Plan) Program and an interdisciplinary care team, this role provides medication reconciliation, patient education, and care coordination; travels to migrant farmworker camps for follow-up visits to close treatment gaps; and partners with the clinic pharmacist to connect uninsured and low-income patients with Medication Assistance Programs (MAPs) to ensure uninterrupted medication access.

Requirements

  • Licensed registered nurse in the state of North Carolina, with maintenance of appropriate certifications, including BLS
  • Native or near-native proficiency in both English and Spanish with excellent oral and written skills in both languages.
  • One year of nursing experience in a primary care or community health setting
  • Strong interpersonal and communication skills, with the ability to work effectively with diverse patient populations
  • Cultural sensitivity, a caring and confident demeanor, flexibility, dependability, excellent written and verbal communication skills, high energy-level, a professional attitude, and desire to serve uninsured/underserved populations
  • Willingness to work in a nontraditional/non-clinical setting, willingness to work late nights

Nice To Haves

  • Experience in community health, integrated care, or with underserved populations
  • Knowledge of farmworker health issues and cultural competency with Latinx communities
  • Previous experience program building, tracking and reporting key performance Indicators to demonstrate program impact
  • Bachelor of Science in Nursing (BSN) or higher
  • Certification in case management or other specialized nursing credentials

Responsibilities

  • Educate patients on prescribed medications — purpose, dosing, administration, side effects, and interactions — and address barriers to adherence (literacy, transportation, language, work schedule) in collaboration with pharmacy, providers, and community health workers.
  • Develop and implement individualized adherence strategies for patients with chronic conditions such as diabetes, hypertension, and cardiovascular disease.
  • Help patients access high-cost medications through Medication Assistance Programs; coordinate with free pharmacies for timely refills to avoid treatment gaps.
  • Partner with the pharmacist on patient medication education; monitor for and proactively address gaps in treatment.
  • Perform medication reconciliation and provide ongoing self-management education for patients in the Care Plan Program, with particular focus on migrant patients.
  • Coordinate care across providers and pharmacy programs; document encounters, care plan support, and outcomes accurately and promptly in the EHR.
  • Participate in interdisciplinary care team meetings to review patient progress and adjust care plans.
  • Travel regularly to migrant and seasonal farmworker camps for chronic disease follow-up visits, assessing living/working conditions related to medication storage and adherence.
  • Build trust with farmworker communities, respecting cultural, linguistic, and logistical considerations; provide on-site medication education, adherence checks, and basic care coordination.
  • Identify patients who become difficult to reach (relocation, job change, etc.) and coordinate strategies to maintain continuity of care.
  • Support clinic nursing services as needed, including triage, nurse visits, and patient education; collaborate and coordinate with the Case Management RN.
