About The Position

Requirements

• Associate degree in Nursing or diploma in Nursing.
• BSN in progress and/or willingness to acquire a BSN within three years of hire.
• At least six months of relevant nursing experience, or active participation in Hackensack Meridian Health's Nurse Residency Program.
• Current and valid Registered Nurse license for the assigned HMH location by the date of hire.
• American Heart Association BLS/CPR certification within 60 days of hire.
• Ability to learn quickly and adapt to changing patient needs.
• Accountability for improving the lives of patients and caregivers.
• Strong teamwork, dedication to ongoing education, and commitment to delivering high-quality, patient-focused care.
• Ability to lift at least 35 pounds, push and pull at least 10 pounds, and stand at least six hours per day.

• National certification in the area of specialty when eligible.

Responsibilities

• Collect comprehensive patient data and perform systematic age- and disease-specific assessments of physiologic, psychological, and cognitive status.
• Recognize changes in clinical and diagnostic status and identify patient and family education needs, including those related to age, disease, and culturally specific learning differences.
• Develop and coordinate prioritized, patient- and family-centered plans of care and goals based on assessment findings.
• Communicate, coordinate, and implement plans of care with patients, families, nursing colleagues, physicians, and other healthcare team members.
• Provide age- and disease-specific care in an organized and timely manner, following established protocols and documenting clinical events.
• Provide or coordinate patient and family education and confirm understanding.
• Deliver nursing treatments that reflect patient needs, advocate appropriately, and comply with evidence-based practice and standards of care.
• Document patient assessments, plans of care, interventions, and evaluations continuously using point-of-care electronic documentation.
• Administer medications using bar-scanning and documentation technologies.
• Coordinate multidisciplinary patient discharges to support safe transitions and continuity of care.
• Evaluate progress toward expected outcomes and use critical thinking to support desired patient outcomes.
• Initiate interventions in response to changes in patient condition or unexpected responses to care.
• Identify ethical issues in the practice setting and bring them to the attention of other team members.
• Recognize and respond to opportunities to improve patient satisfaction.
• Maintain competencies, certifications, and continuing education required by facility accreditation or individual licensing requirements.
• Adhere to the American Nurses Association Code for Nurses with Interpretive Statements and HMH organizational and managerial competencies and standards of behavior.
• Perform other duties and/or projects as assigned.
