About The Position
Baptist Health is seeking a Registered Nurse, Case Manager to join the In-Patient Case Management Department at the Baptist Jacksonville - Downtown campus. This is a Full Time Day shift opportunity, working Monday through Friday. As a Case Manager, you will be responsible for care coordination for hospitalized patients, conducting high-risk screenings and assessments, coordinating post-acute care services with proper documentation, and collaborating with multidisciplinary team members.
Requirements
- Associate's Degree Nursing
- Basic Life Support (BLS)
- Driver's Licensed (DMV)
- Active licensed Registered Nurse (RN)
Responsibilities
- Care coordination for hospitalized patients.
- High risk screenings and assessments.
- Coordinating post-acute care services with proper documentation.
- Collaborating with multidisciplinary team members.
