About The Position
Beacon Hospice, part of Amedisys and the Optum family of businesses, provides home health, hospice, and community-based services. The RN Case Manager coordinates compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans to support each patient’s needs. The role includes on-call responsibilities as assigned. The position offers a salary range of $79,600 to $119,400 annually based on full-time employment, with benefits and incentives subject to eligibility requirements.
Requirements
- Current RN license specific to the state or states where assigned to work.
- At least 1 year of nursing experience.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
Responsibilities
- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
- Develop, prepare, and maintain accurate, timely individualized patient care progress records in accordance with care center policies.
- Record pain and symptom management changes and outcomes as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Communicate regularly with the clinical manager and care team.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform other duties as assigned.
