About The Position

The Hospice Registered Nurse (RN) provides individualized nursing care to hospice patients to maximize comfort and quality of life. The RN works with patients, families, physicians, and an interdisciplinary care team to develop and carry out care plans, and practices with autonomy while receiving support from the organization. The position covers the San Antonio area and is full-time. The nurse supports patients and caregivers, coordinates care across disciplines, and documents patient needs and progress. Participation in on-call or admission rotations may be needed.

Requirements

• Registered Nurse (RN) license in the state of practice.
• At least one year of nursing experience.
• Reliable transportation, a valid driver’s license, and appropriate auto insurance.

Responsibilities

• Provide individualized nursing care that maximizes comfort and quality of life.
• Partner with patients, families, physicians, and the interdisciplinary group to create and carry out care plans.
• Perform comprehensive physical, emotional, social, and spiritual assessments to guide care decisions.
• Educate and support patients and caregivers.
• Coordinate care across disciplines to ensure continuity and dignity.
• Document care clearly and accurately to reflect patient needs and progress.
• Participate in on-call or admission rotations as needed.
• Engage in ongoing learning and professional development.
