About The Position

Beacon Hospice, Amedisys is hiring an RN Hospice Nurse Case Manager to provide hospice nursing care to patients in their homes. The role involves assessing patients, collaborating with the care team to create and implement individualized plans of care, managing and documenting symptoms, and educating patients, families, and caregivers. The nurse also communicates regularly with the clinical manager and care team and performs on-call duties as assigned. The stated salary range is $85,000–$90,000. Compensation may vary by geographic market and by job-related knowledge, skills, and experience.

Requirements

• One year of nursing experience.
• Current RN license specific to the state(s) assigned for work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records accurately and on time, in accordance with care center policies.
• Record pain and symptom management changes and outcomes as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Communicate regularly with the clinical manager and care team.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Perform other duties as assigned.
