About The Position
NCA is seeking a Nurse Case Manager based in St. Paul. The role provides case management services addressing the special needs, care, and quality-of-life challenges of clients, including elders. The Nurse Case Manager collaborates with primary family caregivers, attorneys, financial planners, and trustees who also support the client. This salaried position offers competitive pay, excellent benefits, and a flexible work-from-home schedule. Requirements
- Bachelor of Science in Nursing (BSN).
- At least five years of clinical nursing experience.
- Experience with third-party benefits, including health insurance, Medicaid, Social Security, and Developmental Disability services.
- Excellent written and verbal communication skills.
- Proficiency with MacOS and related software applications, including MS Office.
- Ability to quickly learn and adapt to new technologies and tools in a Mac environment.
- Strong time-management and organizational skills, and comfort working independently in a virtual company or office.
- Ability to collaborate to accomplish goals and work through conflicts.
- Excellent customer service skills.
- Willingness to meet timely documentation requirements.
- Willingness to travel overnight regionally one to two times per month and occasionally nationwide.
Nice To Haves
- CCM or CRRN certification.
- Experience in field case management or workers’ compensation case management.
Responsibilities
- Provide case management services responsive to clients’ special needs and eldercare, care, and quality-of-life challenges.
- Collaborate with primary family caregivers, attorneys, financial planners, and trustees serving the client.
- Research and develop solutions to challenges presented by the client.
- Meet timely documentation requirements.
- Travel overnight regionally one to two times per month and occasionally nationwide.
