About The Position

NCA is seeking a Nurse Case Manager based in Eau Claire. The role provides case management services to address the special needs, care, and quality-of-life challenges of clients with eldercare needs. The Nurse Case Manager collaborates with clients’ primary family caregivers, attorneys, financial planners, and trustees. This is a salaried position with competitive pay, excellent benefits, and a flexible work-from-home schedule. Regional overnight travel is required 1–2 times per month, along with occasional nationwide travel.

Requirements

Bachelor of Science in Nursing (BSN) required.
Minimum of 5 years of clinical nursing experience.
Experience with third-party benefits, including health insurance, Medicaid, Social Security, and Developmental Disability services.
Excellent written and verbal communication skills.
Proficiency with MacOS and related software applications, including MS Office.
Ability to quickly learn and adapt to new technologies and tools in a Mac environment.
Self-motivated and comfortable working in a virtual company or office, with strong time-management and organizational skills.
Demonstrated ability to collaborate to accomplish goals and work through conflicts.
Ability to research and develop solutions to client challenges.
Excellent customer service skills.
Willingness to meet timely documentation requirements.
Willingness to travel regionally overnight 1–2 times per month and occasionally nationwide.

Nice To Haves

CCM or CRRN certification preferred.
Experience in field case management or workers’ compensation case management preferred.
