About The Position

NCA is seeking a Nurse Case Manager based in St. Paul. The role provides case management services addressing special needs and eldercare clients’ care and quality-of-life challenges, while collaborating with primary family caregivers, attorneys, financial planners, and trustees involved in serving the client. This salaried position offers competitive pay, excellent benefits, and a flexible work-from-home schedule. Regional overnight travel is required one to two times per month, with occasional nationwide travel.

Requirements

Bachelor of Science in Nursing (BSN).
At least 5 years of clinical nursing experience.
Experience with third-party benefits, including health insurance, Medicaid, Social Security, and Developmental Disability services.
Excellent written and verbal communication skills.
Proficiency with MacOS and related software applications, including MS Office.
Ability to quickly learn and adapt to new technologies and tools in a Mac environment.
Self-motivation and comfort working in a virtual company or office, with solid time-management and organizational skills.
Ability to collaborate to accomplish goals and work through conflicts.
Excellent customer service skills.
Willingness to meet timely documentation requirements.
Willingness to undertake required regional and occasional nationwide overnight travel.

Nice To Haves

CCM or CRRN certification preferred.
Experience in field case management or workers’ compensation case management preferred.

Responsibilities

Provide case management services responsive to special needs and eldercare clients’ care and quality-of-life challenges.

Research and develop solutions to challenges presented by clients.
