About The Position

Working as a key member of the multidisciplinary team, the Care Manager ensures the effective delivery of patient care through a collaborative approach. The Care Manager interacts daily with all members of the care team to facilitate optimal discharge planning and care coordination. Additional duties include performing admission, outpatient, pre-admission, and continued-stay utilization reviews; completing referrals to community resources; and gathering and communicating relevant information to members of the care team, including patients, physicians, and payors.

Requirements

• Current licensure as a Registered Nurse in Pennsylvania.
• Two years of work experience as a Registered Nurse in an acute care setting.
• Highly effective communication skills.
• Strong negotiation skills.
• Ability to identify and address complex clinical and social issues with patients, families, and members of the multidisciplinary care team.
• Ability to navigate electronic medical records and web-based applications.

Nice To Haves

• Bachelor’s degree in nursing or a health-related field.
• Five years of work experience as a Registered Nurse in an acute care setting.
• Certification in Case Management.
• Experience in care management utilization and/or post-acute care delivery.
