About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The Hospice RN On-Call provides after-hours clinical support, triages patient needs, and coordinates care to support continuity and quality outcomes. The role delivers remote and field-based nursing support, assesses patient conditions, and works with interdisciplinary teams to address urgent and emergent needs. The schedule is on-call Friday, Saturday, and Sunday from 10 a.m. to 10 p.m. The position is based in Milford, Delaware. The posted annual salary range is $79,600 to $119,400 based on full-time employment; pay depends on factors including local labor markets, education, work experience, and certifications. The employer also describes a comprehensive benefits package, incentive and recognition programs, an equity stock purchase plan, and 401(k) contribution, subject to eligibility requirements. Candidates must pass a drug test before beginning employment.

Requirements

  • Current RN license for the state(s) assigned.
  • At least 1 year of nursing experience; hospice, home health, or acute care experience is preferred.
  • Current CPR certification.

Responsibilities

  • Provide on-call nursing support, triage patient needs, and determine appropriate interventions.
  • Assess patient conditions by phone and/or in-person visits and escalate care as needed.
  • Coordinate with physicians, care teams, and caregivers to support timely and effective care delivery.
  • Educate patients and caregivers about symptom management and care plans.
  • Maintain accurate and timely documentation of patient interactions and clinical decisions.
