About The Position

Harbor Hospice is seeking a full-time Registered Nurse (RN) Case Manager for the South Austin area. This is an outpatient position requiring an independent and experienced RN. As an integral member of the interdisciplinary care team, the RN Case Manager admits patients and coordinates appropriate care for each patient and family. Main duties include admissions, assessments, discharges, and pronouncements.

Requirements

• Be a licensed RN in the State of Texas.
• Have effective written and verbal communication skills.
• Be able to travel and have reliable transportation.
• Pass a background check and drug screen, and have a clean driving record.

Nice To Haves

• At least 1 year of experience as an RN.
• Experience with hospice or home health.

Responsibilities

• Admit patients.
• Coordinate appropriate care for each patient and family as part of the interdisciplinary care team.
• Conduct patient assessments.
• Complete discharges.
• Make pronouncements.
