RN Case Manager Hospice (Full-Time)

About The Position

The RN Case Manager Hospice coordinates patient care, continuously assesses the needs of patients and their families, and implements the interdisciplinary care plan. The role provides services in accordance with physician orders and the established plan of care, while ensuring care complies with federal and state regulations. The position requires an active nursing license in the state of practice, at least one year of licensed professional experience, basic technology skills, and the ability to meet specified driving and CPR requirements.

Requirements

Graduate of an approved school of clinical education.
Licensed in the state where the employee practices.
At least one year of experience as a licensed professional.
Basic demonstrated technology skills, including operating a mobile device.
Valid state driver's license.
Maintain automobile liability insurance as required by law.
Maintain dependable transportation in good working condition.
Able to safely drive an automobile in all types of weather conditions.
CPR certification for the healthcare provider.

Nice To Haves

Medicare home health or hospice experience is preferred.
Experience as a licensed professional may be deferred with review and approval from regional and executive service line clinical leadership.
