About The Position

Moments Hospice is seeking an RN Case Manager to provide compassionate, patient- and family-centered hospice care. The RN serves as the clinical lead for patients and their families, managing symptoms, coordinating care, and providing guidance through end-of-life care. The role is field-based, with a Monday–Friday schedule from 8:30 AM to 5:00 PM. A fleet car is available for business and personal use, with fuel and insurance covered at minimal cost. The position requires an active RN license in the state of work, EMR proficiency, and a valid driver's license with a reliable insured vehicle.

Requirements

• Active Registered Nurse (RN) license in the state where the employee works.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• Compassionate, patient- and family-centered approach.
• Valid driver's license and a reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plans of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers about disease progression, symptom management, medications, and what to expect at the end of life.
• Coordinate care across the caseload and serve as the primary liaison among patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to support comprehensive, holistic care.
