About The Position

Paradigm Health is hiring a full-time Hospice RN Case Manager to provide compassionate, high-quality care to patients and families across a local Indiana territory. The role follows a Monday–Friday schedule and includes mileage reimbursement and benefits. The position assesses patients, develops and updates plans of care, provides skilled nursing, coordinates with physicians, and supports patient safety and quality. It also provides oversight to HHA and LPN/LVN staff as needed.

Requirements

Graduate of an accredited nursing program.
Current RN license in Indiana.
Valid state driver's license and reliable automobile.
Proof of current automobile insurance.
Current health certificate or physical examination and TB testing.
Criminal background check.

Nice To Haves

One year of RN experience preferred.

Responsibilities

Assess each patient's condition, initiate a plan of care, and reevaluate and update the plan as necessary.
Provide patient care that considers needs specific to the standard of care for the patient's age.
Demonstrate knowledge of medications and administer them correctly based on the patient's age and clinical condition.
Follow the seven medication rights and reduce the potential for medication errors.
Provide skilled nursing care.
Obtain necessary physician orders and collaborate with physicians as patient needs dictate.
Perform patient care in an environment that optimizes patient safety and reduces the likelihood of medical or healthcare errors.
Support and maintain a culture of safety and quality.
Follow up on lab results and notify the physician of significant results.
Provide oversight to HHA and LPN/LVN staff and co-sign documentation as needed within scope of practice.
Address and support cultural practices when they do not harm others or interfere with the planned course of medical therapy.
