About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The RN Case Manager coordinates compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient’s needs. Pay is based on factors such as local labor markets, education, work experience, and certifications. The stated annual salary range is $75,620 to $113,430 based on full-time employment. Requirements

Current RN license specific to the state or states assigned for work.
Current CPR certification.
At least 1 year of nursing experience.
Valid driver's license, access to reliable transportation, and liability insurance.

Responsibilities

Assess patients and collaborate with the care team to develop and implement a plan of care.
Refer patients to other hospice care disciplines as needed, or document the rationale for not doing so.
Develop, prepare, and maintain accurate and timely individualized patient care progress records in accordance with care center policies.
Record pain and symptom management changes and outcomes as appropriate.
Teach patients, families, and caregivers appropriate patient care techniques.
Communicate regularly with the clinical manager and care team.
Perform on-call responsibilities and provide on-call services to patients and families as assigned.
Perform other duties as assigned.
