Requirements

  • Active RN license (Colorado)
  • Strong clinical judgment and communication skills
  • A genuine passion for end-of-life care and patient advocacy
  • Ability to work independently in a field-based setting
  • Current CPR Certification
  • Valid Driver’s License
  • Valid Auto Insurance

Nice To Haves

  • Hospice or Home Health experience preferred
  • At least 2 years of Hospice RN experience preferred
  • HomeCare HomeBase experience preferred

Responsibilities

  • Provide compassionate, end-of-life care to patients in their homes or care facilities
  • Perform comprehensive assessments and develop individualized care plans
  • Support patients and families with education, comfort measures, and emotional guidance
  • Collaborate with an interdisciplinary team (physicians, social workers, chaplains)
  • Monitor changes in condition and adjust care to ensure comfort and dignity
  • Supervise and guide LVN/LPNs and Hospice Aides
  • Maintain accurate and timely clinical documentation
  • Participate in on-call rotation (if applicable)
