About The Position

Serenity Hospice is seeking a dedicated and compassionate RN Case Manager to provide exceptional care, comfort, and support to hospice patients and their families. The RN Case Manager develops care plans to meet individual needs, conducts regular check-ups, and communicates with family members as needed. The role covers Union, Essex, and Hudson County areas.

Requirements

• Current New Jersey RN license in good standing.
• Willingness to travel to assigned areas.
• Reliable transportation with liability insurance.
• Strong communication and interpersonal skills.

Nice To Haves

• Hospice experience preferred.

Responsibilities

• Complete an initial assessment of the patient and family to determine hospice care needs.
• Regularly re-evaluate patients’ nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient, and family to direct coordinated, continuous care.
• Counsel patients and families in meeting end-of-life needs.
• Initiate appropriate preventative nursing procedures.
• Communicate with the physician regarding patient needs and report changes in the patient’s condition.
• Supervise Hospice Aides and LPNs.
