About The Position

Amador Home Health is seeking a compassionate, organized, and motivated RN Case Manager to serve patients in Jackson, California, and the surrounding Amador County area. The role provides skilled nursing visits and oversees and coordinates patient care throughout each episode, helping ensure patients receive appropriate, timely, and respectful care in their homes. The Case Manager assesses patients’ physical, psychological, and social needs, monitors their condition, and coordinates with physicians, office staff, and other interdisciplinary team members. The position is full-time, day shift, and offers $62–$65 per hour plus $0.67 per mile in reimbursement for eligible work-related travel.

Requirements

Current, valid California RN license.
Strong clinical assessment, communication, organization, and documentation skills.
Ability to coordinate effectively with patients, families, physicians, and interdisciplinary team members.
Commitment to patient-centered care and respect for patient dignity.
Ability to work independently while maintaining strong communication with the care team.
Valid driver’s license.
Reliable personal vehicle.
Current automobile insurance.
Ability to travel to patients’ homes throughout the assigned service area.

Responsibilities

Perform patient assessments and skilled nursing visits.
Complete accurate OASIS assessments and clinical documentation.
Assess pain and implement appropriate pain-management techniques.
Review patient history, diagnostic information, and lab results, and report abnormalities as appropriate.
Maintain an accurate medication profile.
Coordinate care and communicate patient needs with physicians, office staff, and other members of the care team.
Oversee visit frequency throughout the patient’s episode of care.
Participate in weekly case conferences and monthly case manager meetings.
Provide compassionate care that respects each patient’s dignity, rights, and individual needs.
