About The Position

Independence Home Health LLC is seeking a compassionate, dependable, and motivated RN Case Manager to provide quality home health care. The role involves assessing patients, developing and updating individualized plans of care, coordinating care with patients, families, physicians, and other healthcare professionals, and educating patients and caregivers. The RN Case Manager also supervises and supports LPNs, HHAs, and CNAs, maintains complete clinical documentation, and participates in case conferences and quality improvement initiatives. The position is offered as full-time or part-time. Requirements

Graduate of an accredited nursing program.
Active Indiana RN license or a qualifying multistate license recognized by Indiana.
Minimum of 2 years of nursing experience.
Current CPR certification.
Strong clinical, communication, leadership, and organizational skills.
A caring, patient-centered approach and a genuine desire to make a difference.

Home health experience preferred.

Responsibilities

Conduct patient assessments and develop individualized plans of care.
