Requirements

  • Knowledge of nursing principles, practices, and techniques in specialized areas.

  • Knowledge of EHR and Microsoft Office Suite.
  • Skill in the use of nursing equipment and instruments.
  • Valid Florida Driver’s License or the ability to obtain one within 30 days of employment.
  • Licensure as a Registered Professional Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.27.
  • Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency.
  • Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
  • Incumbent may have access to records containing Social Security numbers in the performance of their job duties.
  • Male applicants born on or after October 1, 1962, will not be eligible for hire or promotion unless they are registered with the Selective Services System (SSS) before their 26th birthday, or have a Letter of Registration Exemption from the SSS.
  • The successful candidate will be required to complete the Form I-9 and that information will be verified using the E-Verify system.
  • All Florida Department of Health positions require the incumbent to be able to learn and communicate effectively, orally and in writing, in English. Applicants who do not meet this requirement will not be considered.

Responsibilities

  • Incumbent is a Registered Nurse providing clinical services for clients in adult health/primary care, maternal health, family planning, communicable disease, ADAP, Ryan White and adult/childhood immunizations under the direct supervision of the assigned Senior Community Health Nurse Supervisor at the direction of the Assistant Community Health Nursing Director (ACHDON).
  • Incumbent is expected to follow all local, state, and federal rules and regulations along with complying and performing within the scope of practice as defined by the Florida Nurse Practice Act.
  • At the direction of the ACHDON, incumbent will provide duties at all clinics within Polk County.
  • Preparation of the clinic area and equipment.
  • Interview clients to obtain complete health history and presenting complaints.
  • Take vital signs and perform other screening tests.
  • Follow up on abnormal and late lab reports.
  • Administer medications as ordered by physician/APRN or under medical protocol and guidelines.
  • Make appropriate referrals and assist clients to make necessary arrangements.
  • Maintains accurate records and reports.
  • Provides screening for the HIV antibody virus, pre and post-test counsels, provides education, and refers for medical evaluation and other support services.
  • Document and maintain accurate data entry into the client Electronic Health Record.
  • Works with PFS pharmaceutical system to assist with inventory as needed and notifies any discrepancies to the supervisor.
  • Works independently to provide services to clients under protocol in Family Planning, STD, and/or TB as assigned.
  • Assist physicians and APRNs as needed.
  • Interprets, educates, and reinforces the provider’s plan of care for the client.
  • Collect specimens for laboratory tests and interpret laboratory results.
  • Assess immunization status of all clients and immunize in accordance to current standards.
  • Enters immunization information into Florida Shots.
  • Assessing needs, educating, and planning with families to meet these needs.
  • Giving and/or demonstrating nursing care specific to client’s needs.
  • Providing emotional support to clients and families.
  • Conferring with physicians and other community agencies to provide continuity of care.
  • Coordinates with HMOs regarding appropriate referral services.
  • Participates in ongoing quality improvement activities, including chart audits, peer reviews, performance assessments, and corrective action planning as directed by supervisor.
  • Ensures accuracy, completeness, and timeliness of all client documentation, including Electronic Health Records, immunization records, laboratory data, and case management notes.
  • Monitors clinical workflows and identifies opportunities for improvement, reporting concerns or recommendations to the Senior Community Health Nurse Supervisor or ACHDON.
  • Assists in preparing for internal and external audits, including state program reviews, federal grant monitoring, and accreditation-related evaluations.
  • Participates in root-cause analysis and follow-up when errors, near misses, or adverse events occur, contributing to a culture of safety and continuous improvement.
  • Promotes a culture of excellence, modeling accountability, responsiveness, and adherence to agency values in all quality-related activities.
  • Participates in staff and in-service educational programs.
  • Assists with inventory of supplies, medications, and vaccines.
  • Incumbent will travel to and cover clinics within the county as staffing needs indicate and as directed.
  • Assists with monitoring temperature controls with notation on appropriate logs for refrigerators and freezer per protocol.
  • Maintains time/attendance/travel/EARs records.
  • Maintains and increases competency through educational materials and classes.
  • Assists in orientation of new personnel and volunteers.
  • Assists with preceptorship of students assigned to the clinic.
  • This position may serve clients that speak only Spanish, Creole, or other languages. It may require performance of interpreter duties, if bilingual, or implementation of the Department’s limited English proficiency plan.
  • Performs other duties as required.
