About The Position
Link Rehab and Wellness is seeking a Licensed Clinical Social Worker to join its growing startup. The role offers the opportunity to help shape programs and processes and contribute to patient care and the communities served. Link uses a collaborative, team-based approach and a holistic, patient-centered model focused on each individual’s needs and overall well-being. The position provides behavioral health services, including assessment, treatment planning, therapy, case management, crisis intervention, documentation, and client education.
Requirements
- Licensed in the state of New Jersey as a Licensed Clinical Social Worker (LCSW).
Responsibilities
- Conduct comprehensive behavioral health assessments, including intake interviews, screenings, and evaluations, to determine clients’ mental health and/or substance use needs.
- Develop, implement, and revise individualized treatment plans based on client goals, preferences, and clinical needs, keeping plans aligned with progress and setbacks.
- Provide evidence-based individual, group, and family therapy as appropriate.
- Coordinate and collaborate with psychiatrists, medical doctors, social workers, and community organizations to support comprehensive care and resource utilization.
- Provide crisis intervention, including de-escalation and safety planning, for clients in acute distress or experiencing mental health crises.
- Maintain accurate and timely clinical documentation, including progress notes and treatment plans, in accordance with ethical, legal, and organizational standards.
- Educate clients and families about mental health, coping strategies, and available resources, and encourage participation in treatment and recovery planning.
