About The Position

The Care Manager (CM) manages a small caseload of private-pay clients, coordinating services and resources and providing ongoing monitoring and management in clients’ homes. Clients may also reside in Independent Living, Assisted Living, or Nursing Facilities. The role also supports business development by working with referral sources and building relationships with community and professional organizations that can generate client referrals.

Requirements

Licensed Social Worker.
7–10 years of experience working in geriatrics.
Ability to work with minimal supervision.
Valid driver’s license and dependable personal vehicle.

Responsibilities

Conduct in-depth client assessments covering medical, psychosocial, environmental, financial, legal, and family needs.
Complete a Fall Risk Assessment, Mini Mental Status Examination, and Depression Screening for each client.
Recommend and coordinate services for clients.
Work with clients and families to develop plans of care that address safety and other needs.
Work with clients, families, and professional staff to implement plans of care.
Collaborate with hospital, nursing home, assisted living and memory care staff, physicians, medical providers, attorneys, financial planners, Meals on Wheels, and other geriatric service professionals.
Conduct regular home visits to monitor client status and the home or senior-community environment.
Provide ongoing emotional support and counseling to clients and families as needed.
Provide problem-solving support to clients and fellow care managers as needed.
Provide crisis intervention and use critical-thinking and problem-solving skills.
Work with the business development team to meet prospective clients, families, and referral sources.
Provide backup support to the GCM team as needed.
Track and document time spent on each client to support accurate billing.
Attend educational programs and networking events.
