Hospice Social Worker - PRN

About The Position

The Hospice Social Worker provides psychosocial assessments and counseling services to hospice patients and their families under the direction of the attending physician. The role collaborates with the Interdisciplinary Group/Team (IDG/T) to coordinate care and provide holistic, compassionate support tailored to patients’ and families’ needs. This is a PRN, paid-per-visit position. Compensation is $50 per routine visit, $70 per admission visit, and $35 per hour for grief counseling or crisis management, plus mileage reimbursement.

Requirements

Master’s degree in social work (MSW) from an accredited school of social work.
Current state social worker licensure (LCSW, LMSW, or equivalent).
At least two years of experience in a healthcare setting.
Strong working knowledge of community resources and support services.
Excellent interpersonal and communication skills.
Ability to use computers for documentation and communication.
Compassionate, empathetic, and professional demeanor.
Reliable transportation and valid auto liability insurance.

Nice To Haves

Experience in hospice or palliative care is preferred.

Responsibilities

Conduct psychosocial assessments to evaluate emotional, social, and environmental factors affecting patients and families.
Provide emotional support and counseling to patients and families coping with end-of-life issues.
Coordinate care with the Interdisciplinary Group/Team (IDG/T).
Assist families in understanding and managing grief and loss.
Help families access community resources and services.
Document social work services in accordance with hospice policies and regulatory requirements.
