Licensed Clinical Social Worker
About The Position
Deer Oaks Behavioral Health is seeking a part-time Licensed Clinical Social Worker (LCSW) to provide behavioral health services onsite in one or more long-term care facilities in the clinician’s local community. The role involves therapy, clinical assessment, care planning, and behavioral health support for older adults and individuals with chronic medical conditions. Clinicians have autonomy over their workflow and caseload and receive administrative and clinical support. The schedule is flexible within Monday–Friday, with no evenings, weekends, or on-call work. Deer Oaks is a behavioral health provider focused on long-term care. The organization has served skilled nursing and assisted living facilities since 1992 and works to enhance residents’ emotional well-being and quality of life through compassionate, clinically excellent services.
Requirements
- Active, unrestricted LCSW license.
- Valid driver’s license and reliable transportation.
- Strong written, verbal, and interpersonal communication skills.
- Ability to work independently while collaborating with an interdisciplinary team.
- Comfort navigating long-term care environments.
- Commitment to timely documentation and high-quality care.

Responsibilities
- Provide individual, family, and group therapy to long-term care residents.
- Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
- Support residents experiencing depression, anxiety, adjustment to illness, grief, trauma, substance use, behavioral disturbances, and other conditions common in long-term care settings.
- Deliver behavioral management and brief, goal-oriented interventions aligned with best practices in geriatric care.
- Collaborate with nursing, social services, rehabilitation, and medical providers to coordinate treatment and improve resident outcomes.
- Participate in care plan discussions, facility meetings, and case collaborations as needed.
- Maintain complete and timely documentation using the Geriatric Notes Tool (GNT).
- Follow ethical, professional, and regulatory guidelines for documentation, confidentiality, and care delivery.
- Build professional relationships with facility staff and leadership and serve as a behavioral health resource for residents’ mental and emotional needs.
