About The Position
Windsor Home Health, a division of PREMIER THERAPY, is seeking a Home Health Social Worker (LSW) to provide one-on-one care and help patients remain comfortable at home. The role focuses on enriching patients’ lives and supporting their independence and dignity. Requirements
- Licensed Social Worker (LSW).
- Excellent communication and organizational skills.
- Compassionate, patient-focused mindset.
- Reliable transportation and a valid driver’s license.
Nice To Haves
- Master of Social Work (MSW).
- Home health experience.
Responsibilities
- Assess patients’ social, emotional, and financial needs.
- Assist with community resources and discharge planning.
- Provide counseling and emotional support to patients and families.
- Collaborate with physicians, nurses, and therapy staff to ensure quality patient care.
- Advocate for patient well-being and independence.
